Case Manager – Newport
This position provides information, assistance and comprehensive case management services to individuals or families.  Assesses needs, develops and monitors implementation of service plans, makes appropriate referrals and serves as a customer advocate with other service providers.  It is a full-time position, 32 hours, with a starting pay of $16.05 – $17.36 per hour, D.O.  This position provides care coordination services to eligible individuals and their families to assist them in successful discharge from the hospital.  Works to support their continued health improvement to facilitate a reduction in utilization of emergency services and decrease the recurrence of additional hospitalizations. Administrative Assistant – Republic
Performs routine clerical duties, using independent judgment within established procedures.
